It's Pain.
Look at Pain's databook page and Kishi has it in english as Pain Also, it's fitting with his quotes about feeling pain. Pein is the romanji spelling and i consider it kind of incorrect but thats just me.

It's both.
In japanese, you spell it "Pein" In English, you spell it "Pain" So, you can decide whichever you want to spell it, the "a" or the "e" in Pain/Pein.

wow guys I'm inactive for like, 2 months, I come back and this is what I see in my subscribed threads...
I think we've clarified numerous times that "Pain" is the English version and "Pein" is the literal Japanese translation. now, can we play the quiet game?
