I saw, once that you only need to witness a death (probably a translation error, cause if it was so simple, every Uchiha would have unlocked the mangeyko sharingan before Madara)...as I said in other threads even to you, I guess that there was something or someone, that disabled is Mangeyko sharingan, so Madara needs the bijuu power to unlock it, or a more plausible reason, there was no need yet for him to use the full extent of his power....
