This is not battlegrounds so don`t think too much about feats. Think about how things would really go down from Pain invasion arc until current events. Assume that he is alive. Nothing changes prior to Pain arc, other than him being alive.

What would be the biggest differences?!!

That would make him the only elder who wouldn`t be too scared to fight.
Also, he actually could`ve done something with a good strategy and help from the others.

Sage Mode Naruto would be too late, you say? Not.

The only reason Naruto was late is because Danzo killed that frog/toad. Danzo would not dare to do that if Hiruzen was alive.

And even if he didn`t fight Pain, he`d still do many things.

At least Danzo would not be appointed as a substitute Kage!!! So, he would go to the Kage summit!!!

I like how Danzo was appointed as a kage when in the Pain arc all he did was cause further problems.

No, Hiruzen would not run away from Kage summit like Danzo did. It is mostly his own fault Danzo got himself killed.

Anyway, Sasuke would probably want to kill Hiruzen, too, because he was behind the Uchiha massacre as well.

However, unlike Danzo, Hiruzen would wait in the room along with Ay, Mei, Onoki, Gaara and Mifune and plenty other body guards, including Kakashi and Guy or whoever would be his bodyguard.

Because any jutsu he could potentially use to beat pain would be speculation. Right now I don't see an enma cage or mudwall stopping the gravity thing.

The third would confront pain with no knowledge and would die at Pains Hands... only later to be brought back to life after Nauto has "THE TALK" with Nagato.

Then The Third Would go to Kage summit and HE would be the one leading this war unlike FOOLISH A.
( also Saskue might go to kage summit cuz even though the third did not want Itachi to kill his clan it still happened and Saskue is stupid so he most likely went to the summit and tried to kill Hirzuen but would not because all of the other kages would protect him. Tobi would just save Saskue from distruction)

Then The third would give his life to use Demon Reaper seal to stop EDO MADARA
