So there are a few possible reasons behind this...

Perhaps Kabuto was unable to find Izuna's DNA, thus unable to even revive him.

Perhaps he was too powerful to place under reanimation (for reasons maybe unknown to us)

Maybe he really is still alive, and gave his eyes willingly to Madara because he believed the Uchiha were disgraced and needed revival (remember, Kishi mentioned there'll be a lot of new characters appearing soon). Also remember how Obito was considered deceased but turned up alive. It's plausible that Madara, upon gaining some of hashirama's cells, gave some to Izuna so that he too would also stay alive. We're going to see some more shocks in the next couple of chapters, and it'll be hard to predict them (so anyone who isn't open to this possibility, remember that Kishi can f***k with our minds....

He sucked in B's chakra.
ET revives you the way you were before you died, and Nagato had 0 chakra when he died...that's why he was grey-haired, weak etc.
When he sucked in Bee's chakra he came back to his normal self.

Kabuto said he did extra work on Madara.

Yes, he gave madara his rinnegan eyes, which he didn't have when he died (he took out his eyes I think). And of course, kept the wood style.

with this new chap i am beginning to think Izuna will play a bigger role soon. either as an ET or for a reason behind Madara's plans. i would love for Oro to show up at the battle, than use ET an summon Izuna to face Madara. he would be shocked i am guessing.

I am not even sure if izuna will be revived. He is the only one now worth seeing to be revived, him or the rikkudo sennin aka sage of six paths.

Remember, the power is not in the eyes, but the brain, as Tobirama said. That determined the level of the eye. This is a little hazy on the MS to EMS change, since he came back with his brothers eyes. It's not perfect. That matters more than taking out your rinnegans after the fact.
