Error while canceling an abuse report.

I was writing an abuse report about a double topic post when I decided it wasn't worth reporting. Upon clicking "cancel" I was greated with:
Code:

[phpBB Debug] PHP Notice: in file /report.php on line 25: include(./include/country_check.php.inc) [function.include]: failed to open stream: No such file or directory
[phpBB Debug] PHP Notice: in file /report.php on line 25: include() [function.include]: Failed opening './include/country_check.php.inc' for inclusion (include_path='.:/php/includes:/usr/share/pear:/var/www/viz/shonenjump/include:/var/www/viz/viz/include:/var/www/viz/shopro-usa/include:/var/www/viz/animerica-mag/include:/var/www/viz/shojobeat/include:/var/www/viz/vizeurope/include:/var/www/viz/haikasoru/include:/var/www/viz/vizsignature/include')
[phpBB Debug] PHP Notice: in file /includes/functions.php on line 2119: Cannot modify header information - headers already sent by (output started at /includes/functions.php:3185)

Running the newest version of firefox 3.0.8 (32-bit edition) on vista buisiness 64 bit. Can reproduce the error every time on this Forum but not on any of my personal PHPBB3 forums using a standard user account.

Thanks for the heads up!

We've been having some back end problems, so there's a 99% chance that it's definitely us and not you in any way.

And much respect to anyone besides me brave enough to run Vista at 64 bit.

I have another box running vista 64, hosting web, ftp,SQL, PHP, Exchange, Supporting media and mp3 streaming. No problems at all. 2 Quad cores in it too. Vista isn't as bad as everyone puts out. It isn't good, but still...
