This is pretty close since the distance favors Tsunade. In the end though Orochimaru can still survive her attacks and land a few blows of his own. He seems to have shown better speed after all.

Do they start out with Yamata no Orochi /Katsuyu?

Orochimaru could end this at the start by extending his blade and killing Tsunade. If she tries to heal.....stab her again.

Tsunade could withstand stabs from Orochimaru. She could just pull Orochimaru's tongue and start punching him. The only problem would be how she could pull out his tongue if he
could sometimes spit out his sword.

Well that is true. Tsunade managed to heal a direct hit by Kusanagi in Part 1. She can just grab the sword while it is in her and slam Oro to the ground and then proceed to ground him to a pulp. If and only if, he did not manage to use LAC when he hits the ground

Nah I don't think Tsunade can win here. With his sword, he can basically hack and slash at Tsunade to keep his her down is she heals. Slithering mode makes it so that Tsunade and Katsuya would have trouble getting near him from far range, which is bad because he has long-range diversity and they don't (although he's mostly mid range).

Orochimaru would win, but with a sever injury after that may even lead to death. Tsunade's power is incredible, after all she is the Hokage and one of the Sannin. But Orochimaru has all sorts of evil Jutsu and that deadly sword that killed Sarutobi.

Tsunade was stabbed more than once by orochimaru's sword (not sure if it's kusanagi) and she survived it. I Orochimaru is strong and so is Tsunade but I'm giving Tsunade the edge.
