The sharingan has three states. The normal, tomoe sharingan, the Mangekyou sharingan, and the Eternal Mangakyou sharingan.
The tomoe sharingan has three stages in each eye, to attain the next level, the Mangekyou sharingan, one must acheive a complete set of three tomoe in both eyes.
A one tomoe sharingan has the ability to perceive and see through most Taijutsu, and perceive the flow of chakra, but other than that it has very little power.
A two tomoe sharingan has the ability to copy the Taijutsu that the obe tomoe sharingan could see through, and can see through most Genjutsu and Ninjutsu.
A three tomoe sharingan can perceive all non-Kekkei genkai related Taijutsu, Genjutsu, and Ninjutsu, and copy them accordingly. The user also gains a high-speed teleportation jutsu.
While all of these are indeed seperate forms, it is possible to retain a one tomoe sharingan in one eye and have a two tomoe sharingan in the other. Therefore, all levels tomoe sharingan are just subdivisions of the normal tomoe sharingan, and are considered of the same "state".
The Mangekyou sharingan never changes its appearance, though it does contract when under severe pressure of over-use. It grants the user 3 jutsu, Tsukuyomi, Amaterasu, and Susano'o. It is a singular form, although it is possible to onyl ever master one of these 3 jutsu. Overuse of the mangekyou eventually results in blindness. The appearance is likened to that of a kaleidescope, and Mangekyou sharingan literally means "Kaleidescope sharingan".
In the case of Kakashi, I speculate that, since he only had one sharingan from Obito, the Mangekyou sharingan coukld not completely manifest itself, and therefore offered up a hybrid form which had a different ability altogether.
The final state is the Eternal Mangekyou sharingan, which only Madara Uchiha has ever been said to have completed. It is unknown what powers it grants the user, though we do know that it retains the 3 Mangekyou sharingan abilities, and gains a teleportation jutsu of some sort, which could be an advanced version of the three tomoe sharingans teleportation jutsu. The Mangekyou sharingan can only be attained by taking the eyes of another Uchiha and inserting them into ones eye sockets. The appearamnce of the eternal Mangekyou is a combination of the two Mangekyous, the one of the user, and the one of the person whos eyes have been taken.
In conclusion, I would say that the sharingan has 3 states, the first of which(the tomoe state) has multiple forms within the state.

I agree that the normal Sharingan has three stages. But once Mangekyo Sharingan user recives the Mangekyo, it stays the same stage. Amaterasu (sp?) is just a jutsu used by the Mangekyo Sharingan, it is not a stage. The only other possible explaination is after having the Mangekyo Sharingan, the using forfills the certain condition to change thier Mangekyo to a different kind of eye within the keekei genkai, called the Eternal Sharingan, which -in my opinion so far- is a combination of both the regular and Mangekyo Sharingan.

1) Not true. Each user's Magenkyou greatly differs in appearence, Itachi's, Kakashi's and Sasuke's are all different.
2) Awakening the Magenkyou sharingan does not give you Tsukuyomi, Amateratsu and Susano'o. Those are abilities Itachi gained with his Magenkyou but no one else really had, he GAVE them to Sasuke.
3) How Kakashi awakened his Magenkyou is still a mystery but his jutsu didn't turn out like that because he's limited. Each Magenkyou that's awakened has different abilities and looks different. Kakashi's Magenkyou allowed him to develop his Kamui technique on his own and is compared to the power of Itachi's Magenkyou without him being of Uchiha blood or having two sharingans.
4) We don't know what all of Madara's abilites are yet and there are a lot of theories as to "what" he is since for starters he should be dead either by age or because the 1st "killed him" so stating that because he possesed the Eternal Magenkyou he gained teletransportation isn't really accurate.
