Konohamaru is Sasuke. That's how he knows. (I was wondering about this for a long time too)

It`s simple. Konohamaru`s surname is Sarutobi. Hiruzen`s father was Sasuke Sarutobi. That`s the reason why Sasuke Uchiha`s parents named him Sasuke, so he could once be as great as third Hokage`s father (or Konohamaru`s great grandfather).

Anyway, to answer the question, the same way he knew how Sai looks like!!! :D
