It wasn't confirmed if she died or not.

I think she did die because Kakashi said in chapter 424-425 that He couldn't protect rin, he broke his promise to Obito, please forgive Kakashi.

I think she died during the 9 tails kyuubi attack tragic.

i'm pretty sure rin's dead, otherwise we should see her with kakashi, unless she decides to go travel then that's totally different

well,
once he said he has no one with him, not even a friend to SAsuke
he also said before he died, that he could not protect Rin, he would have said that a long time ago, if she died, maybe she died when Naruto left for training, and that might be how he enables his MS

I thought for MS you had to kill your best friend...

And Kakashi never talks about his past, et wouldn't be unusual for him not to mention it.

Well, you can't always trust Wikipedia, but I do believe that something happened to Rin that killed her.
