There was an episode of Naruto Shippuden, one of the fifties, where Kakashi talked about the elements to Naruto. In it he mentioned how many elements a ninja could use and I was wondering, is it possible for a ninja to use 3 elements at a time?

Do combinations like ice and wood count as elements? If so, then yes ninja can use 3.
Or is it just the original 5: fire, water, wind, earth, lightning??

All 6, there is Water, Fire, Earth, Wind and Lightning, plus the Nature type, or something like that, which only a Rinnegan user can have.
If you mean, can they combine 3 Elements at a time, no singal ninja can do that, unless it is a Kekkei Genkei. I suppose three seperate ninja could combine three different Elements similiar to how you can add wind to fire to super charge it, I don't know what would happen with a 3rd element though.
If you mean, can they Master the use of 3 or more, they said it is nearly impossible to gain all 5 chakra types in a lifetime, but I suppose it is possible, since that girl in the sora filler had all 5, but that was because she stole them from other ninja.

I think that the only ones that count are the fundamental elements: fire, water, wind, earth, lightning. Yamato's mokuton, for example, is just a combination of earth and water. So he really uses two elements, not three.
