Easily, if his sight wasn't failing or his illness killing him. Let's face it, he died from TB (tuberculosis) that flaired up due to his fight with his brother. If he wasn't sick or had good eyesight he would have won!

Itachi meant not to kill him in their fight. His whole aim during their fight was to lure out Orochimaru and then seal him away from his beloved brother...had Itachi wanted he could have easily killed Sasuke.

Actually, it was Madara that awakened Sasuke's Mangekyou Sharingan. To be honest, I never thought Sasuke would actually get it. Don't know why, I figured since he's pretty stubborn and likes to do things his way he would get strong without it.
