I say Shizine pwns,shes a Jonin and is way out of Inos league when it comes to medical ninjutsu,and her poison attacks are deadly enough to kill Ino or Tenten.Its a pity we havent seen anymore of Shizunes attacks :(

Shizune wins. She's easily faster than both of them, and could probably defeat Ino with a metal scapel only (she attacked Kabuto with one, so she knows how to use it :D). Tenten is then overwhelmed by poision needles and poison fog.
