i dont get what it is. what can it really do?

It's not really a genjutsu.

It's a 'you're born with it' kinda thing, and it's used to copy the jutsus of other ninjas for your own use. After Sasuke looks at other people that are throwing flaming balls of mud, for example, he then knows exactly how to do it himself, and how to do it well.

actually their are unique jutsu only possible with mangekyo sharingan usually eye contact directly is required
